Community Concern of Western New York is the leading provider of behavioral health and senior care management services in the Southtowns.  The Mental Health Clinic serves over 900 people annually with a multi disciplinary team of clinical social workers, psychologist, psychiatrist, and certified mental health counselors. We treat a wide spectrum of disorders including marital and family problems, depression, anxiety disorders and acute mental illnesses.

The Senior Care Management Program helps people over age 60 achieve their highest level of independence.  Eldercare specialists help older persons and their families with health insurance, Social Security, entitlements, home energy, home care housing assistance and much more. The program also offers certified personal care, CarePanion and volunteers to help with housekeeping, errands, companionship and more.  Case managers are also certified Care TransitionsTM  coaches that help seniors discharged from the hospital avoid preventable readmissions.
