- Bequest can insure that Community Concern meets the growing demand for behavioral health and senior care services. To provide Community Concern with a bequest, include a statement as simple as: “I give and bequeath the sum of $_________ OR the remainder / or ______% of my estate to Community Concern of WNY, Inc. 6722 Erie Road, Derby, NY 14047 and to be used or disposed of as its Board of Directors deems appropriate.” Please consult with your legal or financial advisor before making any bequests.
