Community Concern continues to be an active participant in the Practice Transformation Academy, which is offered through the National Council for Behavioral Health. Our involvement in the PTA is an important one, as it serves to prepare organizations for the shift from traditional Fee-for-Service to Value-based Payments. Understanding the ongoing shift in the culture of healthcare will allow CCWNY to continue to provide services in alignment with whole-person health, while meeting regulatory changes. In June of 2016 Community Concern was selected to participate, from a group of nation-wide applicants. Our PTA stretch project to improve diabetes self-management.
Megan Brautlacht, LMHC and Debra Henderson, LMHC have been named Behavioral Health Clinic Coordinators for the Derby, and Hamburg sites. With Community Concern’s growth, it was determined that additional leadership and oversight was needed to manage this growth. Megan and Debra have been assets in managing their respective clinics.
Kristy Gasiewicz has been named Program Director for CCWNY’s Health Home Care. Kristy has a decade of experience in Case Management and is eager to service our community. Kristy is located at the Derby facility. Lynn Borgogelli was hired and started this February as a Health Home Care Manager. Lynn is located at the Hamburg location.
Derby, NY - 10/16/2017 - Community Concern of WNY announces that it is acquiring Hamburg Counseling Services to ensure increased access to much needed mental health services in Southern Erie County. An Erie County analysis shows that between 45% and 92% of the population in Southern Erie County have unmet mental health needs.
